How to Request a Letter of Authorization (LOA) - 1Coop Health

1. What is an LOA?

A Letter of Authorization (LOA) is a document issued by 1Coop Health that allows accredited hospitals to provide medical services without requiring you to pay upfront.

2. When do I need an LOA?

You need an LOA whenever you avail of medical services at an accredited hospital under your HMO plan.

3. How do I request an LOA?

Follow these steps:

  1. Go to the Hospital’s HMO/Business Office
    • Present your Certificate of Coverage (COC) and inform them you are a 1Coop Health member.
  2. Submit the Required Documents
    • Certificate of Coverage (COC)
    • Valid ID
    • Doctor’s request or medical order (if applicable)
  3. Hospital Coordination
    • The hospital will contact 1Coop Health to verify your coverage and request the LOA.
  4. Issuance of LOA
    • Once verified, the Letter of Authorization (LOA) will be:
      • Sent directly to the hospital to authorize services without upfront payment, and/or
      • Printed and handed over to the cardholder for their reference and use during the hospital visit.

4. What if the hospital refuses to accept my COC?

If this happens, do not pay immediately. Contact 1Coop Health or inform the Cooperative so we can escalate the case.

5. Can I request reimbursement instead?

Reimbursement is generally not allowed for accredited hospitals. The proper process is to request an LOA before availing of services.

6. Who should I contact for concerns?
